  • Ability to change dentists up to 4 times per year
  • Coverage is effective the first day of the month following enrollment

Specialty Benefits

  • No referral or authorization required for Dental Specialists, chose an in-network dentist and call to schedule your appointment
  • No preauthorization required
  • Covered cosmetic benefits – (DHMO only)
  • Specialty benefits (if copayment does not apply) will be subject to charges (payable by Member), at a percentage of 20% to 25% of dentist’s usual and customary fees

  • No annual maximums for dental work, have as many services as you desire
  • Two or three year rate guarantee options available with written preapproval
