Why can't I view my spouse's claim history?
Privacy laws prevent a person from viewing information on a dependent/spouse over the age of 18. With the member's permission, another person on the policy (age 18 or older) can view the page and even perform functions on the member's behalf. To allow access, members simply call customer service and request access to view with verification.
How do I order an ID card or order a replacement ID card?
You can order a replacement ID card by registering and logging in to your MyPortal Itransact website or by calling customer service specialist.
Can I refer a dentist who is not in your network?
Yes. You can refer a dentist online or call 1-877-493-6282.
Do I pay my deductible to the insurance company or to the dentist?
A deductible is subtracted from any money owed by FCL Dental when a claim is processed

When will I receive my FCL Dental cards and what do they look like?
FCL Dental provides booklets upon your effective date. They will arrive within 10-14 business days upon completion of your new case paperwork. These booklets include a perforated identification card on the back page for your member’s use. Booklets/Cards can be sent to the employee’s home address with receipt of the proper information or to the employer for delivery.

When will I receive my dental renewal?
Dental renewals are sent out 60 days prior to the renewal date.
I would like to add Vision Coverage to my account. What do I do?
Please call your dedicated Sales Representative or Marketing Account Manager for assistance.
Will my members receive a dental card if our program is a DHMO?
Yes, DHMO members will receive a dental card in the State of Texas. Other States will receive a confirmation of Coverage Letter.
How do I refer a dentist to the FCL Dental Network?

A payroll company handles the eligibility for our dental benefit program. Can FCL Dental work with them to manage this process?
Yes, FCL Dental has a dedicated IT Department that will work with your payroll company.
How do I find a dentist?
For Access to an Online Directory of Dentists, Members may visit www.fcldental.com. PPO Members should go to PDP-PPO.
If your plan also uses the DenteMax PPO Network, members may visit www.dentemax.com. Please contact us to ensure you are accessing the correct Plan Network.
Do you take attachments via Electronic Claims (EDI)?
We can accept electronic attachment via EDI, with one of our partnering Tesia Clearinghouse, LLC. ,and we are exploring other vendor services to expand our available options.
You can also send attachment such as x-rays via mail to our address 101 Parkland Blvd. Suite 301, Sugar Land, Texas, 77478. Attention: Claims Department.
I forgot my password online, how can retrieve it?
You can call our Customer Service line at 1-877-493-6282 to assist you in resetting your password.
Do you have a web portal for Dental Providers to submit claims?
Yes we do, you can go to our home page click on Login/Register page to logon to our Itransact Web Portal, once you have register you can submit your claims online including your attachments to us. The claims will be process at real-time and payment will be sent within 30 days. Dental Providers can also submit x-rays attachment when submitting with your claims via the secure web portal access.
Can I go to an out-of-network provider?
No, your dental plan is limited to in-network (INN) providers only. The treating provider must be within the network. To find an in-network provider, visit our provider search tool at https://search.fcldental.com/, enter your zip code, and select your health plan.
I have a dental provider I’ve been seeing for years; how do I get them signed up on the plan?
If you wish to enroll your existing provider in the plan, simply email our provider relations team at pr@fcldental.com. In your email, include the provider’s name, office phone, and location. Once we receive the necessary information, one of our provider relations representatives will reach out to discuss potential network participation.
How do I order an ID card or request a replacement ID card?

Why am I receiving a bill from FCL Dental?
FCL Dental does not bill members directly; all charges are due at the time of service at the dental office. The document you received from FCL Dental is an Explanation of Benefits (EOB). This statement explains the dental treatments and/or services submitted and/or paid for on your behalf.
How can I get a copy of my plan benefits?
For a copy of the benefits covered under your plan, you can email cs@fcldental.com or contact the member services department at 1 (8XX) XXX-XXXX.
